It is a single formal certificate, separate from the transcript (bang diem) that lists the actual courses and grades. Older diplomas may use different seals and layouts depending on the issuing university and the year.

Common pitfalls on this document
Classification bands, not loose words
Xep loai Gioi or Kha must read as a defined band, not as a vague word like good. The band is what the evaluator needs to weigh the result.
Degree kept faithful
The Vietnamese degree and major are not remapped to a US degree name. Mapping is the credential evaluator job, not the translator job.
Diploma is not the transcript
The diploma proves the award; it does not list courses or grades. If course detail is needed, the separate transcript must also be translated.
